I'm confused by your question.  OTR clients encrypt and decrypt
messages such that servers see only ciphertext.  For a federated
protocol, like XMPP, the encrypted messages should pass through
multiple servers and arrive at the destination client.  So in that
sense OTR messages are already capable of being sent through multiple
servers, and there is no need to alter the OTR protocol to support
this.
